- Professional Qualifications Certificates.
- Work Experience.
- References (Name, Addresses, email and Telephone Number of 2 people who can provide you with references. This must be a charge nurse/sister/senior level).
- DBS Disclosure Certificate.
- Bank details.
- P45 or equivalent.
- Proof of professional registration.
- NMC PIN NO.
- Proof of immunisation — Hepatitis B, Rubella, Measles, Varicella and TB Certificates.
- Next of kin details.
- GP details.
Senior Healthcare & Care Assistants Registration Requirement
- 2 passport size photographs.
- Proof of Identity (Passport / Birth Certificate / Driving licence. British citizenship certificate or indefinite leave to remain (ID/letter) that shows if you have legal rights to work in UK).
- National Insurance Number.
- Eligibility To Work in the UK.
- Professional qualifications.
- Relevant experience & references (Name, Addresses, email and Telephone Number of 2 people who can provide you with references. This must be a senior level staff).
- Minimum 2 years work experience.
- DBS Certificate.
- Bank details.
- P45 or equivalent.
